TO REGISTER FOR THE RN FIRST ASSISTANT PROGRAM

To register for this course, you must submit all THREE of the forms (Registration, Letter of Recommendation and Preceptor) to RN First Assistant. To attain these forms, you have two available choices:

• Fill out and submit the on-line form provided below (simply fill out the Registration form and click "Submit") to have further information and forms mailed to you, or;
  
• Download and print out the MS Word file available below. This MS Word file has the Registration Form, Letter of Recommendation Form and the Preceptor Form. You can print out the forms from your MS Word program and mail them in to the address listed on our Contact page.
